7 Things I Check Before Placing Any Medical Equipment Order

If you're the person who gets handed the requisition forms and told "we need this yesterday," you know exactly what I'm talking about.

I've been managing medical supply orders for a sleep medicine clinic for about four years now. We're not huge—fortyish employees across two locations—but we order a lot: CPAP devices, masks, accessories, lab supplies, and the constant restocking of clinical essentials. My purchasing budget runs roughly $400,000 a year across six or seven vendors.

I didn't start out with a system. I learned by making mistakes. Some of those mistakes cost me money. One cost me a conversation with my VP that I'd rather not repeat.

Here's the checklist I now run through before every single order. It's seven steps, and most of them have nothing to do with finding the lowest price.

Step 1: Get the actual clinical need in writing

A nurse saying "we need a new CPAP machine" isn't enough. What problem are they actually trying to solve? Is it the device itself, or is it a mask fit issue?

This sounds basic, but it's where I got burned early on. A clinician put in a request for "resmed masks for sale"—they were literally googling it, screen in hand. I ordered a bulk set of full-face masks from the first result. Turns out the patient needed a nasal pillow instead. Those full-face masks sat in the storage closet for eight months until the respiratory therapist finally used them for staff training.

Now I ask three questions before ordering anything:

  • What's the patient condition or use case?
  • What parts and accessories are compatible with the devices we already own?
  • Is this a replacement, an upgrade, or a brand-new setup?

Write it down, get it in an email. Not because you don't trust your colleagues—but because verbal requests get forgotten when the clinic gets busy. (Should mention: this also protects you when someone claims later they asked for a different model.)

Step 2: Verify compatibility like it's your job (because it is)

This is probably the most expensive lesson I've learned in this role. If a product description says "compatible with most CPAP machines," treat that as a red flag, not a green one. Verify the model number yourself. It takes five minutes and saves a lot of uncomfortable conversations.

Step 3: Check the regulatory category before you commit

Not every medical supply is regulated the same way, and understanding what category you're buying into changes how you vet the vendor.

Take "what is in vitro diagnostics"—this comes up more often than you'd think in procurement conversations. IVD products are tests performed on biological samples like blood, urine, or tissue, outside the body. Common examples include blood glucose strips, rapid antigen tests, and chemistry analyzers. They follow a different regulatory pathway than therapeutic devices. Some IVD tests need FDA clearance or approval for a specific intended use; others are waived or moderate complexity, which affects how they're bought and used.

Before ordering any diagnostic or testing product, I confirm the vendor can show me the regulatory clearance documentation. For the U.S. market, that typically means an FDA 510(k) or similar authorization. According to FDA guidance (fda.gov), medical devices and IVD tests are classified by risk, and classification determines the compliance requirements. If a vendor can't point me to that paperwork, it's a deal-breaker—no matter how good the price is.

The same logic applies to therapeutic equipment. I verify the device model is cleared for U.S. distribution and record that authorization in the purchase file. It takes a few minutes and gives me a defensible paper trail if an auditor ever asks why we bought what we bought.

Step 4: Calculate the total cost, not the sticker price

The lowest per-unit price is rarely the lowest cost. It isn't even close.

Take IV catheters. They're a high-turnover consumable in any clinic that does infusions or blood draws. A box of 100 might range from around $12 to $50 depending on brand, gauge size, and safety features. But the real driver is usage efficiency. If the nursing team needs retraining on a new system, or the chosen catheter has a higher failed-insertion rate, that small price difference vanishes overnight.

We had a similar experience with electronic pipettes. I chose a budget model because it was about 30% cheaper than the market leaders. Then we discovered the calibration cycle was stricter, replacement batteries weren't standard, and the lab techs found the grip uncomfortable. Four months later, we bought the ones they'd actually asked for. If you ask me, an instrument that lab staff use hundreds of times a day should be selected by the people using it—not by whichever spreadsheet line looks best.

So glad we caught that one early, by the way. We were one purchase order away from standardizing a pipette model that would have frustrated the entire lab team for years.

Total cost means: upfront price + consumables + training time + maintenance + replacement risk. Some of those factors don't show up on the quote. You have to add them yourself.

Step 5: Decide what your deadline is worth—before you need the deadline

Here's an opinion I've formed after four years of medical supply ordering: rush fees buy certainty, not just speed. And sometimes, certainty is precisely what you need.

In the spring of 2023, a physician walked into my office three days before a visiting specialist's clinic day. She wanted a specific electronic pipette in time for a test panel we were running that week. Standard shipping would arrive in five business days. Overnight wasn't possible because of battery transport restrictions on that unit. We paid an extra $180 for guaranteed delivery from a supplier with a same-day dispatch record. Was it annoying to explain the premium to finance? A little. Did I sleep better that week? Absolutely.

I've had the opposite experience too. A vendor promised "probably on time" delivery of critical mask fittings two days before we needed them. They arrived four days late. That cheaper, vague-quote choice cost us a rescheduled patient and about eight phone calls I don't want back.

In my opinion, the issue isn't paying more. It's being unable to quantify the risk when "probably" goes wrong. Missing a clinical deadline can cascade into rescheduled procedures, unhappy physicians, and patient complaints. Once you frame it that way, the rush fee is often the cheapest line item in the order.

Step 6: Insist on documentation before the first dollar is spent

I hate that I learned this one the hard way.

A few years back, I found a supplier with excellent prices on a bulk order of disposables—about 20% cheaper than our regular vendor. I placed the order. Then the invoice arrived, handwritten, with no lot numbers and no expiry date information. Finance rejected the expense report. The materials were already in use by then, which made everything messier. That one mistake cost me more than the savings—and it made me look careless to my VP.

The rule now is simple: proper invoicing, lot numbers, expiry tracking, and any requested certificates are non-negotiable. I verify a vendor's documentation capability before placing the first order, not after. If they can't show me what a standard invoice looks like in advance, they don't get the purchase order.

Take it from someone who ate $2,400 in rejected expenses: the extra ten minutes of paperwork verification is the cheapest insurance you will ever buy.

Step 7: Keep a running inventory, even for the boring supplies

The items nobody thinks about until they're gone: IV catheters, pipette tips, tubing, filters, gloves. When I took over purchasing in 2020, I didn't track any of them closely. It wasn't until a three-week backorder on a common catheter size that I built a simple stock sheet—just a spreadsheet with reorder points and lead times. It's not fancy. It now saves my department about 10 hours a month in last-minute sourcing.

That spreadsheet paid for itself again when our primary supplier had a shortage in 2024. Because we'd tested a backup vendor earlier, switching some orders over took one phone call instead of a panic. Part of me wants to consolidate to a single vendor for simplicity. Another part knows that redundancy saved us during that shortage. I compromise with a primary-plus-backup model and review it every six months.

If you're managing orders for a clinic, keeping eyes on the high-volume consumables is honestly more valuable than negotiating a 5% discount on the big-ticket items.

Three mistakes I see all the time now

  1. Assuming "medical grade" means the same across categories. It doesn't. A device's regulatory pathway, its compatibility requirements, and its supply chain risk profile are completely different from a diagnostic test's or a consumable's. Know what category you're buying before you evaluate vendors.
  2. Not building buffer time for specialty items. A standard box-stock CPAP mask can ship quickly. But a specific size from a specific mask line, or a battery backup with particular output ratings, might take weeks to source. Add lead time into the ordering plan, not just the product price.
  3. Leaving the end users out of the selection. A nurse who inserts catheters daily has strong opinions about which ones work. Lab techs have strong opinions about pipettes. Ask them before ordering, or get ready for a reorder sooner than expected.

This checklist isn't complicated. It's thorough in a way that feels boring when you're busy at 4:00 PM on a Tuesday. But if you've ever had to explain to a physician why their requested item is backordered because someone skipped a compatibility check, you know exactly why the boring version is worth it.

In my experience, good procurement in a clinical setting is less about finding the magic supplier and more about asking the same five questions every single time. The discipline is the strategy. That, and always getting it in writing.
