Used ResMed CPAP Machine: A Buyer’s Guide for Sleep Clinics & Home Care Providers

There’s no single right answer for buying a used ResMed CPAP machine

I’ve been managing procurement for a 50-person sleep clinic network for about six years now—overseeing roughly $180,000 in cumulative equipment spending across things like CPAP units, masks, and accessories. One question I get asked more than any other: “Should we buy a used ResMed machine?” And the honest answer is: it depends completely on your situation.

People assume the lowest price always wins. What they don’t see is which costs are being hidden or deferred—repairs, missing parts, shorter lifespan. From the outside, a $400 used AirSense 10 looks like a huge savings over the $900 new price. The reality is sometimes you’re buying a problem with a motor count of 8,000+ hours and no warranty.

So instead of giving one piece of advice, I’ll break this into three scenarios. Figure out which one fits you, and go from there.

Scenario A: Budget is the primary constraint

If your clinic is just starting out, or you’re managing a very lean home-care program with patients who can’t afford new equipment, a used ResMed CPAP machine can make sense—but you have to know exactly what you’re getting.

In 2023, I compared costs across 6 vendors for used AirSense 10 AutoSets. Vendor A quoted $450 per unit, including a 90-day warranty and a free replacement mask. Vendor B quoted $350—seemed like a better deal. I almost went with B until I calculated the total cost: they charged $85 for a 6-month warranty extension (doesn’t cover motor failure), $40 for a “reconditioning fee” they didn’t disclose upfront, and $25 for basic cleaning supplies. Total with one-year coverage: $500. Vendor A’s $450 included everything. That’s about 10% difference hidden in fine print.

If you’re in this scenario:

  • Always ask what’s NOT included before you ask the price.
  • Get the motor hours. Anything under 5,000 hours is a decent starting point; under 3,000 is ideal.
  • Look for a vendor that lists the run time, original model number (AirSense 10 vs. 11), and whether it includes the original humidifier. That kind of transparency usually means they’re not hiding surprises.

My recommendation: If the used unit is under 3,000 hours, comes with a 90-day warranty, and costs less than 50% of new price, it’s worth considering—especially for patients on a fixed budget. Just budget an extra $100 for possible repairs later.

Scenario B: Quality & patient adherence are non-negotiable

If you’re running a higher-volume sleep center or you’re a DME provider that relies on compliance data (and reimbursements tied to adherence), going used can be riskier. Every spreadsheet analysis pointed to the cheaper option for my clinic in 2022—we needed 12 units fast. Something felt off about the vendor’s response time. Turns out that ‘slow to reply’ was a preview of ‘slow to deliver.’ Two units arrived with cracked tubing connectors, and one had a motor that was audibly louder than it should be.

I still kick myself for not checking the vendor’s refurbishment process. If I’d asked for documentation on cleaning methods and replaced parts, I could have avoided a reorder that cost us $1,200 in expedited shipping and lost patient trust.

Analysis based on industry standards for medical device refurbishment:

  • Used units should undergo cleaning per FDA guidelines for reusable devices—including disinfection of exposed surfaces.
  • At a minimum, consumables like filters, tubing, and headgear should be replaced.
  • The machine should be tested for pressure accuracy ±1 cmH₂O (per AASM standards).

My recommendation: Don’t buy used unless the seller provides a detailed refurbishment checklist and a minimum 6-month warranty on the motor. The cost savings aren’t worth the risk of a failed unit affecting patient adherence rates.

Scenario C: The ‘best overall value’—where used makes sense if done right

The numbers said go with the cheapest new model from a direct-to-consumer brand. My gut said stay with ResMed because of their myAir platform and ecosystem compatibility. I’m not going to pretend I always get it perfect. One of my biggest regrets from early in my role: not investing in a vendor relationship early. The goodwill I’ve built with one particular supplier over 3 years means they now give me first access to trade-in programs and refurbished units before they hit the open market.

If you have some budget flexibility—say you can spend 60-70% of new price—you can get a near-new unit that’s essentially the same as a factory reconditioned model.

What to look for in this scenario:

  • Unit refurbished by a ResMed-authorized service center (not a third-party refurbisher using aftermarket parts).
  • Motor hours under 1,500 (often these are returns from short-term rentals).
  • Full accessories included: heated humidifier, power supply, travel case, SD card.
  • Warranty of at least 12 months on the entire unit.

My recommendation: In this scenario, a used ResMed CPAP machine can save you 30-40% versus new while still providing 95% of the lifespan and performance. I’ve done it myself for our clinic’s overflow inventory.

How to decide which scenario you’re in

Answer these three questions honestly:

  1. What’s your maximum budget per unit? If under $500, you’re in Scenario A. If $600-$800, consider Scenario C. If over $900, buy new.
  2. How important is warranty support? If you need at least 6 months of full coverage, don’t go below Scenario B standards.
  3. What’s the motor hour threshold you’ll accept? If you can’t verify hours or the seller won’t disclose them, walk away. That’s a red flag.

I’ve learned to ask “what’s NOT included” before “what’s the price.” The vendor who lists all fees upfront—even if the total looks higher—usually costs less in the end. That’s true whether you’re buying used or new.
